Patient Responsibilities

Referrals/Coverage
We request that you contact your insurance company prior to your initial visit to determine what your financial and paperwork responsibilities are for physical therapy. 
While our Patient Relations Team works hard to help you understand your healthcare coverage, there are too many insurance plans for our office staff to keep track of. 


Payment
Unfortunately most insurance plans do not provide full coverage for physical therapy and patients have some financial responsibility for your care. We kindly ask for all payments to be made at the time of service. 
Please determine if your insurance plan has, and how much you are responsible for:

  • Co-Payment
  • Deductible
  • Co-Insurance
We accept cash, check, credit and debit cards (excluding American Express) and Health Savings cards (HSA).


 

What is needed for the first Appointment?
  1. Insurance card
  2. Prescription from referring doctor (if required by your insurance)
  3. Verification of co-payment, deductible or co-insurance amount from your insurance company
